So what is the All-On-FourR procedure?
To put it simply, the All-On-FourR procedure allows us to place a full arch of teeth on only 4 or more implants. The dental implants are small, pure titanium fixtures that are placed into the jawbone. As the best option available for treating missing teeth, implants are used to support single tooth implants, or bridges of a segment of teeth. Usually, single implants for single teeth require several months to heal in the jaw after insertion due to their need to individually stabilise a single tooth in a row of existing teeth. With the All-On-FourR protocol, the use of as little as 4 implants allows us the ability to immediately fit a prosthesis to the fixtures rather than experience the long waiting time usually required of single implants.
The back implants are inserted at an angle to ensure stable support for total rehabilitation. Placing these implants on an angle helps overcome any bone deficiency or weakness often found in the back of the jaw as the result of gum disease or severe wear. Doing so helps us avoid the need for bone grafting.
If you think of the 4 implants together forming a stable foundation, a full arch of teeth is then installed and fixed into place. Step 1: Consultation with your expert
In this consultation, we sit down with you to assess your particular dental concerns and outline a treatment plan to address them. We will look at your medical and dental history in great detail to develop the best understanding of your unique requirements.
Step 2: DSD & X-Rays
Following your first consult, we may take photographs, 3D models, and diagnostic radiographs and full X-Rays to form a thorough picture of your case. Digital Smile Design comes into play as an integral phase in your treatment to optimise the aesthetic outcome of your result.
We are the stand alone practice currently in Australia that incorporates DSD (Digital Smile Design) into our All-On-FourR treatment plan. Digital Smile Design is a technology that allows us to digitally design and pre-plan your smile on your face before going ahead with treatment. It allows us to work with you to design the end result you’re happy with before making any changes to your smile. Combining All-On-FourR with DSD allows us to ensure that your investment results in the perfect smile for you.
Step 3 Meeting your surgeon:
Once we’ve agreed on your best dental plan, we proceed with the first day – the surgical appointment. This will be your surgical appointment. After meeting your surgeon, Dr. Jaclyn Wong, she places your implants for where your teeth will be anchored. We perform this procedure under local anaesthetic with the option for you to be under sedation or general anaesthetic. Therefore, you have the option of being completely asleep during your procedure. We insert your new teeth and fix them to your dental implants. Once that’s done, you can go back out in the world as a new person, enjoying the confidence that your new teeth will restore for you.

Dr. Paltoglou is the principal dentist at The Melbourne Smile Centre. He commenced practice in 1987 and holds a Bachelor of Dental Science from the University of Melbourne. He is a member of the ADA (Australian Dental Association), the American Academy of Cosmetic Dentistry, the European Academy of Aesthetic Dentistry and the Australian Society of Implant Dentistry.

Dr Jaclyn Wong
Dr. Wong is our oral surgeon. Her services include wisdom teeth extractions, complex extractions, implant surgery, pathology, All-on-4™, exposures and pre-prosthetic surgery. Jaclyn has trained with experts in the field of All-on-4™ surgery, travelling to Portugal to learn from the inventor of the system at the Malo Clinic.
Jaclyn has completed both medical and dental degrees at the University of Melbourne, and has trained in general and specialty surgery with the Royal Australasian College of Surgeons. After graduating with Honours in Dentistry she worked as a registrar for six years at The Royal Melbourne Hospital, Royal Dental Hospital and Austin Hospital.

Why do you need initial teeth?
There are two key reasons why you can’t have your final teeth placed immediately:
- Your gums will continue to change and absorb the impact of your initial teeth’s placement for up to a year. As such, you will need to reline your new dentures. When your original bridge(s) are placed, it will take some time to get used to the new way you speak and chew. Your bite will also adjust to the change. As we review your progress, we will make notes and reassess the fit and function of your bridge. Any necessary changes will be amended in your final teeth.
- After we place your implants, your jawbone moulds itself to the implants. This process is called osseointegration. Acrylic bridges have the ability to absorb the impact of your bite more gently which helps the osseointegration process. Due to its high rigidity, your final bridge absorbs less of your bite’s force which slows the healing process on your jaw. That’s why we place a more forgiving acrylic bridge first to allow your jaw to get used to your full set of teeth.
The standard option for All-On-4™ treatment involves fitting an initial acrylic fixed bridge. As a guideline, this fixed bridge lasts between three to five years before needing replacement. Many patients choose to replace the initial fixed bridge with a long-term restorative solution made of zirconia or porcelain.
What Are The Differences Between All-On-4 And Dental Implants?
Both All-On-Four® and dental implants require surgical placement of an implant into your jawbone. These fixed implants then support fixed or removable dentures and crowns.
The key difference between both relates to the number of implants for replacement teeth that are installed. Replacing an entire set of teeth with dental implants is a complicated and lengthy process. If each tooth were individually implanted you would require jaw bone grafting and many months of healing before your mouth would be ready for crowns and dentures. For an implant to integrate with your jaw, we need to allow months of time for them to heal and stabilise through fusion with your existing bone and gums. With All-On-Four®, a full arch of teeth is installed to 4 free standing implants that anchor them in place so the fusion process can occur without movement. The healing time is much faster with All-On-Four® because there are fewer implants and we are able to place a temporary denture on them whilst your permanent denture is prepared.
If you are looking at replacing all or most of your teeth, the All-On-Four® procedure is more cost effective than individual implants. It also reduces the impact on your life from healing time, and the total number of dental visits required
What Are The Differences Between All-On-4 and Removable Dentures?
The most important difference between All-On-Four™ and dentures is that the first option allows you to have fixed, non-removable teeth. That means that you won’t need to remove your prosthesis after eating, or sleeping. Instead, you will be able to care for your All-On-Four™ prosthesis very similarly to how you would look after your natural teeth.
You also won’t have to worry about denture adhesive with All-On-Four®. Since your bridge is attached to implants, you are free to live your life without worrying about fixing your teeth. Unlike removable dentures, you won’t experience wobbling, slipping or any movement with All-On-Four®.
We often advise patients with removable dentures to avoid certain foods that are tough to chew or bite into. With All-On-Four® you can eat whatever you wish exactly as you would with your natural teeth.
Once placed, your All-On-Four® implants can last decades if taken care of. This removes the inconvenience faced with removable dentures where patients will need to have their dentures refitted every few years.
The All-On-Four® bridge is fixed only to where your teeth need to be. This avoids the discomfort of having your palate covered like with a removable denture. This results in less irritation to your gums and tissue, and no altered sense of unfavourable taste or temperature to your mouth. It’s also great for patients with gag reflex issues.
What Are The Risks With All-On-4?
All-On-Four® implants has a very high success rate. The success rate for dental implants is around 95% with an almost 100% success for bridges.
However, any procedure will have potential risks and complications. With All-On-Four™ they include:
- General surgical complications: infection, poor healing, extensive bleeding, or discomfort
- The rare instance that the dental implant fails
- Dental implants not integrating due to things such as smoking, poor bone density, nutrition or a pre-existing health condition
As important as the success of our implementing the procedure is your ability to maintain the correct care for your All-On-Four® implants. Things that can impact your success rate includes:
- Poor oral hygiene post treatment
- Allowing a build-up of plaque which leads to bone loss, gum disease and implant failure
- Smoking which prevents healing
Teeth grinding which will cause wear that affects the longevity of your bridge.
Are The Risks Associated With All-On-4 Preventable?
The risks associated with All-On-Four® are easily preventable. Implants can fail due to peri-implantitis (gum disease that affects the success of an implant). You can avoid peri-implantitis with proper care which involves regular brushing, flossing and check-ups with us. Patients that are prone to peri-implantitis are those that poorly manage their plaque build up, have pre-existing periodontitis that has become chronic, partial edentia, smoking addiction. If you have a pre-existing dental condition that may affect your All-On-Four treatment, you will need to flag this with us. Often these things can be dealt with pre-treatment or through careful maintenance under our guidance. Discussing your medical history prior will help us ensure the success of your outcome and ongoing health.
Does All-On-4 Hurt?
As with any surgical procedure, you will experience some discomfort during the healing process after surgery. During the procedure, we will administer a general aesthetic (allowing you to fall asleep) or IV sedation (where you are in partial sleep). This ensures that the treatment is as pain-free as possible. The All-On-Four® procedure is considered minimally invasive and involves no bone grafting. This allows a faster recovery time than with traditional implants.
What's the recovery time for full arch implants?
Your recovery after All-On-Four® will be quicker than with standard implants. You will have fully functioning teeth in 24 hours. Despite this it can still take between 3-6 months for full recovery.
After your surgery, we will provide you with gauze to help control any potential bleeding from your implant sites. You will be advised to change this every 30 minutes until your bleeding is finished. Aside from gently rinsing your mouth with either water or the rinse we provide you, we recommend that you avoid touching or physically impacting your mouth during the first 24 hours.
To improve your recovery rate we also recommend:
- Avoiding any hard foods
- Getting rest
- Avoiding strenuous activities
How Long does All-On-4 Last?
All-On-4® dental implants can last decades. The longevity of your All-On-4® treatment can vary depending on your age, the quality of your implants, your health, and whether you floss and brush your teeth properly.
